Dear Employee:

I hope this letter finds you well and that your time away from work has been helpful. I am writing to ask for your help by letting us know what your plans are at this time regarding your return to work and to remind you of several University services and programs that you may find beneficial.

Our records indicate that you have been absent from the office since April 24. 2XXX. We also have on file a completed Physician's Certification form and an approved Request for Family and Medical Leave requesting an absence from work through June 14, 2XXX, As of June 14, 2XXX, our records indicate that you will have a sick leave balance of approximately 12 hours and will have exhausted your 480-hours of Family and Medical Leave. You will not be eligible for another 480-hours of Family and Medical Leave until December 4. 2XXX.

Based on these materials, we expect you to return to work at your regularly scheduled start time on June 15, 2XXX. Please contact me immediately if you believe you are going to unable to return to work as planned so that [make arrangements to cover your position and so that you and I can discuss what, if any, options we have available to us at this time, Failure to report to work on June 15, 2XXX or otherwise contact me beforehand to discuss other arrangements will be considered a failure to maintain a work schedule and may be subject to corrective action up to and including dismissal.

The University offers its employees a number of services and programs that you may find beneficial at this time. The University Employees Assistance Program provides employees and their family members with confidential, professional short-term counseling services when they have personal problems that may affect their work performance and general well being. You may contact them at 472-3101 for more information and details.

If you believe you may have a medical condition that will affect your job performance when you return to work, I would strongly encourage you to contact Jill Flagel, the University's Director of Faculty and Staff Disability Services at 472-3417. Jill works with individuals with health problems that affect their ability to meet job expectations to determine what, if any, accommodations may be made available to them under the Americans With Disabilities Act.
