Please read before completing forms:
The forms below may only be submitted by currently enrolled Arts and Sciences students. After filling them out, obtaining the necessary signatures and gathering the appropriate documents, submit these forms to the Arts and Sciences Advising Center. A copy of the request form with the College’s decision will be mailed to the student. Please bear in mind that submitting a request does not guarantee approval.

Chief Undergraduate Major Advisers and Professional Academic Advisers will make recommendations regarding individual requests for substitution and waiver of courses toward general degree, major and minor requirements. The Arts and Sciences Advising Center takes such requests under advisement using the guidelines established by the College Curriculum Committee. Requests for Substitution and/or Waiver of Arts and Sciences requirements should be turned in to the Arts and Sciences Advising Center, 107 Oldfather Hall, P.O. Box 880330, Lincoln, Nebraska, 68588-0330.

Request for Substitution of Major, Minor or Equivalency

  • Use when requesting substitution of UNL course(s) for a major or minor requirement
  • Use when requesting substitution of transfer course(s) for a major or minor requirement
  • Use when requesting that an equivalency with UNL course be established for a transfer course
  • Must include major or minor adviser’s signature

Request for Substitution of Essential Studies
Request for Substitution of Essential Studies
  • Use when asking that a transfer course apply as elective, Essential Studies, or upper level credit
  • Use when asking that a transfer course apply toward an entrance requirement
  • Use when requesting that a UNL special topics course apply toward an Essential Studies requirement if the course is not on the approved list. See the Undergraduate Bulletin for the listing of approved Essential Studies courses
  • Use when requesting pre-approval for a transfer course you plan to take for general degree credit
  • Must include documentation (ex: syllabus, letter of support)

  • Use when requesting to be waived from completing a requirement as listed in the Undergraduate Bulletin
  • Must include documentation (ex: syllabus, letter of support)
  • Must include major adviser’s signature

Other Forms:
The following forms are processed by advisers as they are received in the Arts and Sciences Advising Center in 107 Oldfather Hall. After filling them out, return them to the Advising Center and an adviser will instruct you on completing the necessary steps for your request.

Proposal for an Individualized Program of Studies

  • Please read the guidelines of the IPS program before beginning your proposal
  • Must include several signatures and letters of documentation as described in the guidelines
  • Requests are submitted to the Advising Center and are reviewed by the College Curriculum Committee on a monthly basis

  • Use when requesting to take more than the allowable number of credit hours in a term
  • Please see the guidelines on the number of credit hours the College of Arts and Sciences will allow
  • Must include signature of major adviser

119 Pre-Professional Status Form
119 Pre-Profesional Status Form
  • May be used by students from any UNL college who want to declare a Pre-Professional area of study
  • The College of Arts and Sciences advises for Pre-Law and several Pre-Health areas

Military Adacemic Plan: Major/Minor Checksheet
Military Academic Plan
  • Please read complete instructions before completing form
  • Attach this form to your Army 104-R, Air Force Form 48, or Navy Military Academic Plan
  • You may need more than one form if work does not fit on one page
  • Must include major adviser’s signature verifying all major(s) coursework
  • Must include minor adviser’s signature if you have declared minor(s)
  • Check your general education requirements with Bachelor of Arts or Bachelor of Science checksheets (located in 107 Oldfather Hall)
  • Please allow 48 hours for an adviser to review your plan.
